There are four Setup options:

Fresh install installs SMD for the first time.
Install installs the same SMD version to newly added Domino server(s).
Upgrade upgrades an existing SMD installation to the latest version or build.
Install and Upgrade installs SMD to additional Domino server(s) and upgrades an existing SMD installation to the latest version or build.
